Becoming a patient

​

Dr. Ou is currently accepting new patients by application only. This is to ensure that there is a good fit on both sides. The average wait time for a consultation is usually less than 2 weeks.

​

To reserve your appointment, we ask for a $400 deposit, which is fully refundable if you cancel at least two business days in advance. This is to discourage people from making appointments then not showing up, which is unfair to other patients who have been waiting for their appointments.

Please note:

  • Dr. Ou does not offer long distance consultations

  • He accepts children on a case by case basis.

  • He does not accept insurance.

​If you'd like to become a patient, please start by downloading the Patient Application and completing only the first step.  Then wait to hear back from our office before finishing the rest.

​

Most people find it easiest to print the form, fill it out by hand, and email it to info@bridgestohealthatl.com.  Once your application is approved, you can fill out the new patient paperwork (last updated June 16, 2023)
